Primary Care Mental Health Practitioner Team

The Primary Care Mental Health Practitioner Team supports adults experiencing common mental health difficulties that do not require specialist secondary mental health services. The team works with you to understand your needs and help you access the right support.

Appointments can take place at home, in a clinic, at your GP surgery or by telephone where appropriate.

  • Appointments: Monday - Friday : 9:00 am - 5:00 pm
  • Telephone: Monday - Friday : 12:00 am - 12:00 am

Team phone is available 8am-4pm and does not accept text messages.

Who is the service for?

• Adults aged 18 and over.
• People experiencing common mental health difficulties who have been referred by a healthcare professional.

• Mental health assessments.
• Advice and short-term support.
• Help to find the most appropriate service for your needs.
• Support for people who may find it difficult to access services.
• Medication advice and reviews where appropriate.
• Employment support to help you find or stay in work.
• Peer support from people with lived experience.

What we don't offer

• Crisis or emergency mental health support.
• Mental health diagnosis.

The team will make reasonable adjustments wherever possible to help you access the service. Please let them know if you have any additional needs.

Waiting times & timescales

You will usually be offered an assessment within 2–4 weeks of your referral.
